have any trouble viewing or downloading the forms that need to be
returned, please contact the school office. If needed, we can make
arrangements for you to pick up paper copies. Thank you!
- The "Information Forms" contain important information that you will need to read. The forms marked with (***) require signatures on the "Signature Page" as confirmation that the information was read.
- The "Forms to be Completed and Returned" need to be printed, filled out and returned to school. Some need to be completed for each child, and some are one per family.
- The "Optional Health Forms"
include various medical forms for those students who have medical
needs. Please fill out medical forms as needed for each child.
- The "New Student Enrollment Forms" are for students who are new to our school.
PRESCHOOL PARENTS: The only forms that you will need to read or fill out and return to school are marked with a (Preschool too). You are also required to read the Preschool Handbook and the Parent Student Handbook, and sign the Signature Page that you have read both handbooks. If you want to be included in the School Directory or your child has a medical issue, please fill out the Optional Forms that apply to your situation.
